Horizon's Home

Horizons Home is an Adult Foster Home that focuses on MI clients with some DD tendencies. They are geared towards independence and helping their patients attain their greatest potential. There is a 24 hour plan of care. Unsupervised living occurs in 4-6 hour segments allowing for personal independence. They encourage individuals to work through ODC, DAC, or other agencies that provides support/direction in the workplace. They also encourage you to consider their home for clients capable of continuing their education at Bemidji State University or Northwest Technical College. Within the Beltrami county area there are many services provided for 12 Step programs, Chemical Dependency, Anger Management and a variety of other support groups. Transportation is set up or provided depending on the need. GRH and CADI are requested (other waivers may be accepted depending on the individual and the need).

3 bedrooms located downstairs with a full kitchen set up, full living room area, and washer/dryer.
