Medication Therapy Management

Medication Therapy Management Services (MTMS or simply MTM) is drug therapy management furnished by a pharmacist. The Medicare Prescription Drug, Improvement, and Modernization Act of 2003 included a provision for Part D eligible individuals to qualify for MTM if they meet the following criteria:
- Have multiple chronic diseases
- Are taking multiple drugs
- Are likely to incur expenses that exceed a specified level
MTM is a patient specific, individualized service provided directly from a pharmacist to a patient. At MedSave, we feel that these sessions can play a vital role in meeting the healthcare needs of individuals. We are so committed to this program that in the spring of 2007 we underwent a major remodel of our store in order to provide more space for our pharmacists to prepare and meet with patients.
Some of the topics you can look forward to learning more about during an MTM session include:
- Drug-drug interactions (Including prescription, non-prescription, and herbal medications)
- Drug-condition interactions (Does any of your drug therapy adversely affect your other medical conditions?)
- Appropriate doses (Is the dosage you are taking appropriate for your current condition?)
- Unnecessary therapy (Are you taking a medication that you no longer need?)
- Missing drug therapy (Is there a prescription or other medication missing from your daily regimen?)
- Compliance issues (Are you having problems taking your medications the way your doctor intended?)
- Proper monitoring of drug therapy (Are you having the labs and other follow-up you need to make sure your drug therapy is safe and effective?)
- Therapeutic duplication (Are you on more than one medication that works the same way?)
- Potential cost savings when appropriate (Are you aware that in some cases another drug with the same ability to treat your condition, but with a lower price tag, may be available?)
In addition to the eligibility of Medicare Part D qualified individuals, the State of Minnesota also has their own MTM program for those individuals who are receiving benefits through the state Medicaid program. We also have pharmacists on staff who are qualified to provide those services.
